College Application Records Request

As part of the college application process, students need to request various records from the GRS College and Career Counselor (Matt McElrath).  The form below has been created to streamline this process. Students, please click here to complete the request form  for each school to which you are applying–whether the school uses the Common App, SENDedu, or good old fashioned US mail to collect application information. You will need to log in with your GRS email account to access the form. Incomplete forms, or records requested in a format other than this form, will result in a delay in getting your records request fulfilled. Thanks for helping us help you.
